COVID 19 Travel Advisory/Forms

Dear NYSAR members,

In response to Executive Order #205, individuals traveling to New York from an “impacted jurisdiction” are required to quarantine for 14 days. It is NYSAR’s recommendation that licensees ask every client/customer/individual whether they have traveled to/from any other state since 12:01a.m. June 25, 2020. If the client/customer/individual answers ‘yes’, the licensee should inquire which state(s) and reference the Department of Health (DOH) website that lists the “impacted jurisdictions” where such mandatory quarantine would be required (the website is provided below).

If the client/customer/individual traveled to/from any “impacted jurisdiction” since 12:01a.m., June 25, 2020, the licensee should not conduct any type of showing or in-person activity with the client/customer/individual until the mandatory 14-day quarantine has ended.

NYSAR sent an update yesterday and created a new form that can be utilized in support of the update. NYSAR has added an additional question to the “COVID-19 HEALTH SCREENING QUESTIONNAIRE” and renamed it the “COVID-19 HEALTH SCREENING & TRAVEL ADVISORY QUESTIONNAIRE”. NYSAR recommends replacing the original screening questionnaire with the new one so licensees also inquire about clients/customers/individuals traveling to/from other states.
