There have been no changes to the agency disclosure form. DOS merely placed a fillable form on their website and changed around the original formatting. The form is statutory in nature and there is no real official form. So long as the form being used contains the language required under the statute, it meets the requirements. Font size, checks vs. lines etc…
Many boards (including NYSAR) and brokerages have made their own form containing the required language that are differing font sizes, check boxes, fill in the circle etc… all are perfectly legal. Licensees have never been required to use the DOS form. In fact the forms on NYSAR.com are not the DOS forms but ones that I drafted.
There is no update to provide to members since the form has not changed.
